چکیده
A 30-year-old woman presented with back pain, weakness and numbness in both lower extremities, and difficulty initiating voiding. The patient was diagnosed at age 3 with beta-thalassemia intermedia. Physical examination revealed frontal bossing, a prominent mandible, T 4-level tenderness, decreased strength, light touch, pin prick and temperature in both lower extremities, abdomen and back with a T7-T9 level , and decreased vibratory and position sense in both toes. Bilirubin was 2.3/0.3 mg% with a serum iron of 241 . Emergency magnetic resonance (MR) was performed of the entire spine (Figs. 1 and 2). This was followed by MR of the brain (Fig. 3). Intravenous dexamethasone and radiation therapy was initiated emergently. The patient's lower extremity strength improved and sensation was regained. One year later, she returned with similar complaints and MR demonstrated a recurrent posterior epidural mass compressing the spinal cord from T6-T9. Discussion

Extramedullary Hematopoiesis in a Patient with Transfusion Dependent Beta-Thalassemia Presenting with Cord Compression
Extramedullary hematopoiesis (EMH) is a physiological compensatory phenomenon occurring in many hemolytic anemias including thalassemia. Besides transfusion, radiotherapy, surgery or a combination of these modalities, hydroxyurea (HU) as an optimal treatment has been described occasionally. We described a case of beta-thalassemia major who has been on regular blood transfusion and developed EMH...
متن کاملMR of thoracic cord compression caused by epidural extramedullary hematopoiesis in myelodysplastic syndrome.
Spinal cord compression caused by extramedullary hematopoiesis is a rare complication of chronic anemic states, most frequently occurring in patients with beta-thalassemia. We report the MR appearance of extramedullary hematopoiesis resulting in cord compression in a patient with a myelodysplastic syndrome, which was isointense with the spinal cord on T1-weighted images and markedly hypointense...
متن کاملSpinal cord compression due to intraspinal extramedullary hematopoiesis in thalassemia intermedia.
Extramedullary hematopoiesis is a common phenomenon in thalassemia. During the disease there are very rare occasions when compensatory hematopoietic tissue is located in the intraspinal epidural space, causing spinal cord compression. This complication requires urgent neurosurgical consideration and decision for further treatment. We present a case of thoracic spinal cord compression secondary ...
متن کاملParavertebral Extramedullary Hematopoiesis In A Pregnant Thalassemic Patient: A Case Report
Spinal cord compression due to extramedullary hematopoiesis is a well-described and rare syndrome encountered in several hematological disorders including β-thalassemia. Hereby, a 37-year old pregnant woman with intermediate β -thalassemia with paraparesis and lower limb hypoesthesia was presented. MRI s...
متن کاملCord Compression due to Extramedullary Hematopoiesis in an Adolescent with Known Beta Thalassemia Major.
We describe a 16 year-old male with β thalassemia major and gait disturbances that had not been given blood transfusions due to a severe childhood transfusion reaction. Thoracic spine MRI demonstrated hematopoietic marrow throughout the spine and epidural masses causing cord compression consistent with extramedullary hematopoiesis (EMH). After treatment with steroids, radiotherapy and monitored...
